Volunteer Acknowledgment, Assumption of Risk & Release
I certify that the information I have provided is true and complete to the best of my knowledge. I understand that Central Florida East Coast Charities (“CFEC”) may verify information provided and may decline, reassign, or end my volunteer service at any time. My participation is voluntary and does not create an employment relationship or entitlement to wages or compensation.
I agree to follow the instructions of CFEC and event staff, perform volunteer duties in a safe and appropriate manner, report injuries or unsafe conditions, and not volunteer while impaired by alcohol, illegal drugs, or any substance that prevents me from safely performing my duties.
I understand that volunteer activities may involve ordinary risks, including lifting or carrying items, slips, trips or falls, crowds, vehicle/loading activity, weather exposure, and interaction with members of the public. I knowingly and voluntarily assume the risks associated with my participation. To the fullest extent permitted by law, I release and hold harmless Central Florida East Coast Charities (“CFEC”), the Central Florida East Coast Toy Drive, Space Coast Harley-Davidson, and their respective directors, officers, employees, volunteers, agents, participating organizations, sponsors, advertisers, and, where applicable, owners or lessors of the event premises, from claims arising from my participation, except to the extent a claim cannot lawfully be released.
In an emergency, I authorize CFEC or event personnel to contact emergency services and my emergency contact as reasonably appropriate. I understand CFEC does not undertake to provide medical care.
Photo, Video & Media Release
I understand that the Toy Drive is a public event where photography, video recording, and media coverage may occur. By volunteering, I grant Central Florida East Coast Charities and the Central Florida East Coast Toy Drive permission to photograph, record, and use my name, image, likeness, voice, and statements in print, digital, social media, website, video, television, promotional, fundraising, and other materials related to Central Florida East Coast Charities and the Toy Drive, without compensation to me.
Parent / Legal Guardian Consent — Minor Volunteer
Required: The volunteer will be under 18 on December 5, 2026. A parent or legal guardian must complete this section before the form can be submitted.
I certify that I am the parent or legal guardian of the minor volunteer named above and consent to the minor's participation in volunteer duties associated with the 2026 Central Florida East Coast Toy Drive, held December 5 & 6, 2026. I understand that CFEC and participating organizations are not responsible for providing individual adult supervision for my child.
I have read the Volunteer Acknowledgment, Assumption of Risk & Release above. I understand the nature of the volunteer activities and, on behalf of myself and to the extent permitted by law, authorize the minor's participation and acknowledge and assume the risks associated with that participation.
Minor Media Authorization: I authorize CFEC and the Central Florida East Coast Toy Drive to photograph or record the minor volunteer and to use the minor's name, image, likeness, voice, and statements for the same purposes described in the Photo, Video & Media Release above, without compensation.
