I understand that CFEC Charities may use the information provided to determine eligibility, coordinate assistance, prevent duplicate holiday assistance, and administer the toy drive. CFEC Charities may verify limited eligibility information with participating organizations when reasonably necessary. Assistance is based on available resources and is not guaranteed. I agree to notify CFEC Charities if another organization provides holiday assistance for a child listed in this application.

CFEC Charities uses this information to review eligibility, coordinate holiday assistance, arrange distribution, prevent duplicate awards, and handle appeals or exceptions. Access should be limited to authorized workers with a legitimate program need. Do not submit Social Security numbers, bank account numbers, detailed medical records, or protected shelter-location information on this application.
